This Community Health Needs Assessment was carried out in 2013 to identify current and future healthcare needs in the community and to improve and strengthen programs and services provided to address them. They surveyed residents in the service area and local community agencies, and compiled this report which addresses areas of interest, including cancer, diabetes, and prenatal care.
The purpose of this report is to provide a framework to implement critical technology that will enhance the state’s overall economic competitiveness through informed decision-making, increased transparency and managed investment. Excel Bridgeport is a movement of parents, students, and other community members to advocate for excellent schools. We believe that going to college and having successful careers should be the expectation and not the exception for Bridgeport children. To get there, we need to focus on 1) Achievement, 2) Good Governance, 3) Effective Leadership, & 4) Community Empowerment. This report provides a summary of 2013 student achievement data, in order to equip parents and community members to be informed advocates and suggest strategies for moving forward.
This Community Health Needs Assessment was carried out in order for Griffin Hospital to gain a better understanding of the Valley community around it. Based on the results, the hospital will improve the services it provides.
Bristol Hospital led a comprehensive Community Health Needs Assessment (CHNA) to evaluate the health needs of individuals living in and around Bristol, Connecticut beginning in 2013. The purpose of the assessment was to gather information about local health needs and health behaviors. The assessment examined a variety of indicators including risky health behaviors (alcohol use, tobacco use) and chronic health conditions (diabetes, heart disease).The completion of the CHNA enabled Bristol Hospital to take an in-depth look at its greater community.
