  • Waterbury 2013 Community Health Needs Assessment

    The Greater Waterbury Health Improvement Partnership led a comprehensive Community Health Needs Assessment (CHNA) to evaluate the health needs of individuals living in and around Waterbury, Connecticut beginning in 2012. The partnership consisted of Saint Mary’s Hospital, Waterbury Hospital, Waterbury Department of Public Health, the City of Waterbury, the StayWell Health Center, the Connecticut Community Foundation, the United Way, and other community partners. The purpose of the assessment was to gather information about local health needs and health behaviors.

  • 2013 Middlesex County Community Health Needs Assessment

    For eighteen months, a broad coalition representing Middlesex County worked to produce this county health needs assessment with a focus on chronic disease. The initiative is part of the Centers for Disease Control and Prevention Community Transformation Grant project here in Connecticut. Through oversight from the Connecticut Department of Public Health and Chatham Health District as project lead and grant fiduciary, the assessment reviewed demographics, health indicators,health disparities, and health assets, and performed a policy and environmental scan.

  • Greenwich Hospital Community Health Needs Assessment Final Report 2013

    Beginning in September 2012, Greenwich Hospital led a comprehensive Community Health Needs Assessment (CHNA) to evaluate the health needs of individuals living in the hospital’s Service Area.  The purpose of the assessment was to gather information about local health needs and health behaviors. The assessment examined a variety of indicators including risky health behaviors (alcohol use, tobacco use, etc.) and chronic health conditions (diabetes, heart disease, etc.).
